Jasmine Lazaro

Jasmine heals in multiple languages and listens in all of them. She is fluent in English and Tagalog and conversational in Spanish, not because it looks good on paper, but because she has sat across from people who could not find the words for their pain in a language that wasn't theirs, and she decided that would never happen in her care. She graduated from Charles R. Drew University of Medicine and Science in California, an institution founded in the wake of the Watts uprising with a singular mission: to serve the underserved. That was not a coincidence. That was a choice that tells you everything you need to know about who she is and how she practices.
Jasmine is a board-certified family nurse practitioner and a certified wound care clinician with years of experience across primary care, chronic disease management, and pain management. She has worked in hospital systems and clinical settings across the Las Vegas valley, treating patients of every age, background, and walk of life. But she did not build Releaf to replicate what already exists. She built it because she saw what was missing. She saw patients cycling through fifteen-minute appointments with providers who never learned their names. She saw people choosing between groceries and a copay. She saw a system that was technically functioning but failing the people inside of it.
At Releaf, nothing is rushed. Jasmine conducts comprehensive assessments. She takes the time to understand not just the symptoms but the life surrounding them, the stress, the sleep, the work schedule, the things a patient might never think to mention, but that shape everything about their health. She builds individualized care plans rooted in evidence-based medicine and integrative approaches because she knows that healing the body without addressing the mind and the spirit is only doing part of the job. She specializes in wound care and pain management because those are the places where people are most often dismissed, most often undertreated, and most often told to just live with it. She refuses to accept that.
Celebrating 14 Years in Healthcare

Tomi Love
BSN, RN
Tomi was seventeen years old when she enrolled in a certified nursing assistant program. Seventeen. Most people at that age are figuring out who they want to be. Tomi already knew. She completed her certification at eighteen and stepped onto a hospital floor for the first time, learning what it means to show up for someone in their most vulnerable moments. That was not the beginning of a career. That was the beginning of a calling that has not stopped moving forward since.
Over the next decade she built one of the most well-rounded clinical foundations you will find anywhere. She rose through phlebotomy, laboratory leadership, acute care, and nursing, earning the highest-tier lab assistant position at Sonora Medical Center and being recognized with a Service Excellence Award for the kind of care that made an entire department better. She went on to earn her Bachelor of Science in Nursing from Chamberlain College of Nursing while building a business on adaptability and sheer will.
At Releaf, she conducts assessments, supports treatment plans, drives quality improvement, and handles the invisible labor that keeps a practice not just running but running with integrity. She does not separate leadership from service. To her, they are the same thing.
She has led outreach in places most providers would never go, organized community classes, trained volunteers, and testified before government committees about public health failures she witnessed firsthand. She is currently pursuing her graduate education to become a nurse practitioner because she is not done. She cares for her ninety-two-year-old father at home, which has taught her more about dignity and advocacy than any textbook ever could. Tomi believes healthcare changes when people on the ground decide to build something better and then actually do it.Tomi has never waited for conditions to be perfect. She builds in the conditions she is given. That is what she has always been doing. And she is just getting started.
